Healthy Connections: How to Apply & Deadlines
The Request for Proposals provides an overview of the initiative, eligibility criteria, and an outline of the application process. To apply for a Healthy Connections grant we recommend the following steps:
- Read through the Request for Proposals
- Complete the HIT Self-Assessment Tool*
- Complete the RFP along with the required Attachments and Appendices
*Note: Collaboration grant requests are not required to complete the HIT Self-Assessment Tool
We will accept proposals that are e-mailed, mailed or delivered to our office at the address below or sent electronically to Sara Pasquariello prior to one of our four deadlines:
- October 15th, 2008
- January 15th, 2009
- April 15th, 2009
- July 15th, 2009
