Healthy Connections: Grant Types

The initiative will support all phases of HIT and Health Information Exchange (HIE) assessment, planning and implementation for both individual health care organizations, as well as communities of providers, clinics or health care organizations. We will award three types of grants:

  • Assessment and Planning Grants. These grants are for individual primary health care organizations that are currently developing a long-term vision for HIT and HIE and assessing how to move forward. Selected organizations will receive consulting support through an HIT/HIE assessment and planning process. Upon completing the assessment and planning process, organizations may apply for implementation grants in future grant cycles. Grantees will be expected to dedicate staff time to the organizational assessment and technology planning process, and provide periodic reports, and participate in the HIT learning community on an ongoing basis.
  • Implementation Grants. These grants are for individual primary health care organizations who are already implementing comprehensive HIT plans.
  • Collaboration Grants. These grants are for collaborative partnerships, consortia or networks of organizations representing health care providers/clinics in their community or region on planning, implementing or expanding HIT and HIE. Both planning and implementation funding will be available.
