Grant Guidelines

Through our grantmaking, we seek partners who will help make Colorado the healthiest state in the nation by promoting our community outcome areas of Healthy Living, Health Coverage and Health Care.

We have established 12 Measurable Results that align with the three outcomes listed above. Grants will be awarded to organizations that can demonstrate the capacity to achieve these results.

Using the Measurable Results, we will regularly report our progress, sharing both successes and projects that didn't go as planned with our grantees. We will use the lessons learned from the evaluations to adapt our grantmaking.
