Healthy Connections: Grant Types

The initiative supports all phases of HIT and Health Information Exchange (HIE) assessment, planning and implementation for individual health care organizations, as well as for communities of providers, clinics and health care organizations. We award three types of grants:

  • Assessment and Planning. These grants are for individual primary health care organizations that are currently developing a long-term vision for HIT and HIE and are determining how to move forward. Select organizations will receive consulting support through an HIT assessment and planning process. After completing the assessment and planning process, organizations may apply for implementation grants in future grant cycles.
  • Implementation. These grants are for individual primary health care organizations that are already implementing comprehensive HIT plans. 
  • Collaboration. These grants are for collaborative partnerships, consortia or networks of organizations representing health care providers/clinics in their community or region for planning, implementing or expanding HIT and HIE. Both planning and implementation funds are available.
