Healthy Connections: Strengthening Health Care through Information Technology
Healthy Connections is a multiyear initiative designed to improve the health information technology (HIT) capacity of providers serving low-income, uninsured Coloradans by:
- Making the health care system more efficient
- Reducing the rate of preventable medical errors
- Closing gaps in the quality of care for underserved populations
Although there is a nationwide ten-year effort underway to achieve universal adoption of electronic health records (EHRs) and to develop a system for sharing health information, nonprofit providers lack the necessary resources to fully participate in this effort.
Healthy Connections supports organizations that are just beginning to consider how HIT can support their missions, as well as those on the cutting edge of HIT implementation. Healthy Connections is designed to expand the use of HIT in Colorado’s safety net by:
- Providing grants for HIT assessment and planning
- Providing grants for implementing HIT, including technical assistance
- Promoting collaborations of providers using HIT
- Communicating best practices and lessons from the Healthy Connections initiative
