Special Funding Opportunities

We have developed Special Funding Opportunities for grants and projects the Foundation has identified as high impact. Some of these opportunities are ongoing initiatives, while others are designed to address specific needs within a designated timeframe.

Currently the Foundation is seeking applications in the following areas:

School-Based Health Care
School-based health care is an effective strategy for reaching underserved students. Recognizing the important role school-based health care programs play in the health and wellness of Colorado's children, the Foundation launched a $10.8 million initiative in June 2009 to support school-based health care in communities throughout Colorado. Long-Term Services and Supports (LTSS)
A balanced LTSS system will ensure people with disabilities and chronic conditions have choice, control and access to a full array of quality services that assure optimal outcomes, such as independence, health, quality of life and community integration. The Foundation is releasing a special funding opportunity to support Colorado in designing a high-quality, cost-effective and person-centered LTSS system. Learn more

Older Adult Subsidy and Medicare Savings Programs
To increase enrollment of eligible adults in the Low-Income Subsidy Program (LIS) and the Medicare Savings Programs (MSP), the Foundation seeks to fund organizations that can demonstrate cost-effective strategies for identifying and enrolling qualified Medicare beneficiaries with limited incomes and resources in LIS and MSP. Increasing Enrollment of Eligible Coloradans in Medicaid and CHP+
Health insurance is critical to an individual's ability to access health care. Despite the availability of public insurance programs, thousands of Coloradans are uninsured, including approximately 176,000 children. Specifically, the Foundation seeks to fund two-year projects to build partnerships between safety net organizations and county departments of human/social services or other state-designated medical assistance sites to out-station enrollment technicians at clinic sites. Increasing Enrollment of Eligible Students in Medicaid and CHP+
The Colorado Health Foundation invites organizations to submit applications to increase the enrollment of eligible students in Medicaid and the Child Health Plan Plus (CHP+). Specifically, the Foundation seeks to fund two-year grants to build partnerships between school districts and county departments of human/social services or other state-designated medical assistance sites.
