The Colorado Health Foundation collects personal information when you register or use the Foundation's websites or register for an event with the Colorado Health Foundation, when you use the Colorado Health Foundation contact form or when you use the "join our mailing list" form.
When you register for certain events we may ask for information such as your name, email address, phone number, mailing address, zip code, occupation, title, and organizational affiliation.
The Colorado Health Foundation automatically receives and records information on our server logs from your browser, including your IP address, and the page(s) you request.
The Colorado Health Foundation uses information for the following general purposes: to fulfill your requests for products and services, improve our services, contact you, conduct research, and provide anonymous reporting for internal and external clients.
Information Sharing and Disclosure
The Colorado Health Foundation does not rent, sell, or share personal information about you with other people or nonaffiliated companies or organizations.
