Funding Strategy: Health Information Technology

Grant Examples

If all Coloradans have access to an electronic health record with data that can be shared between providers, then they will receive higher-quality coordinated care and take a more proactive role in managing their own health.

The Colorado Health Foundation invests its resources in the following areas to increase the adoption of health information technology in Colorado:

  • Provide safety net clinics, such as federally qualified health centers, rural health clinics, community mental health centers and community-funded safety net clinics,with the resources to plan and implement electronic health records and/or disease registries by:
    • Promoting quality improvement and practice redesign through the use of health information technology
    • Encouraging uses of health information technology that promote patient-centered care and active participation in establishing plans and achieving outcomes
    • Supporting community collaborations aimed at sharing best practices and lessons learned for health information technology implementation and use
  • Expand health information exchange for all health care providers in the state of Colorado by:
    • Facilitating local health care providers connecting to local health information exchanges
    • Supporting communities that choose to connect to the statewide health information exchange
    • Providing startup capital to the statewide health information exchange, the Colorado Regional Health Information Organization
    • Providing expansion funding to local health information exchanges
    • Funding safety net clinics to connect to health information exchanges
    • Encouraging health information exchanges to include safety net clinics in planning and implementation
  • Build leadership and community support for health information exchange and sharing of clinical data across health care settings by:
    • Communicating the benefits of health information exchange to our grantees and key partners
    • Working collaboratively with health care stakeholders
