Colorado Regional Health Information Organization and Quality Health Network
A total of $20.4 million in grants from the Foundation supports these organizations in building a health information exchange network that will ensure providers have access to secure, comprehensive health data for individuals across Colorado.

High Plains Community Health Center
High Plains received a two-year grant to upgrade technology and support transformation of how they deliver services to manage clinical outcomes, support quality-improvement plans and train new and existing staff in the optimization of HIT systems.

North Colorado Health Alliance
A partnership of safety net health care providers in Northern Colorado, the North Colorado Health Alliance augmented its local HIT system to include three new organizations and added comprehensive behavioral health to the mix of services to improve quality and support better integrated care.

Clinica Family Health Services
Clinica received a two-year grant from the Foundation to enhance the use of electronic health records as a tool for clinical quality improvement. Clinica serves more than 35,000 low-income and other underserved people in Boulder, Broomfield and Adams counties. The clinic uses a model of care that integrates primary, dental and mental health services.
