Grant Examples: Optimize Coverage in Public Programs

Examples of grants in this Funding Strategy include:

Family and Intercultural Resource Center – Through school-based outreach activities, a collaborative effort with the Summit County School District aims to increase enrollment for students eligible for Child Health Plan Plus (CHP+) and Medicaid. The program uses free and reduced lunch data to target enrollees.

Together Colorado – This group works to expand, defend, sustain and implement affordable, accountable health coverage for Coloradans. Together Colorado also educates the public about benefits becoming available through national and state legislation initiatives.

Colorado SOAR Project – This project is intended to strengthen Social Security Disability and Medicaid benefit acquisition for those who are homeless or at risk of homelessness in Colorado. SOAR is a partnership between the Colorado Community and Interagency Council on Homelessness and the Colorado Behavioral Healthcare Council.

Boulder County Housing and Human Services – With support from a Foundation grant, the department developed a tool kit and training program focused on Colorado PEAK, the state’s new online Medicaid/CHP+ application. The project will allow counties and community organizations to leverage Colorado PEAK to its full potential.
