Clínica Tepeyac: Patient Portal

Implement a patient portal that will support low-income, Spanish-speaking community members who are often at risk of or suffering from chronic disease in managing their health. Clínica Tepeyac will utilize its best practice-based approach to clinic in-reach and patient education to enroll patients in the portal and train them in its use--increasing patient activation and enhancing the relationship between patients and providers by offering a flexible, patient-centric tool that allows working poor community members to access their personal health information at their convenience.
