Funding Opportunity: Advancing Team-based Care through Technical Assistance
In light of the coronavirus (COVID-19) pandemic and its far-reaching impacts across the state, this funding opportunity has been modified to meet nonprofits where they are in this new landscape.
This funding opportunity will support safety net clinics in receiving technical assistance (TA) to improve their clinical and business capacity so they can offer high-quality, team-based comprehensive primary care. The Foundation will fund clinics up to $125,000 over the two-year period to support clinic staff participation in the funding opportunity and any education and travel-related expenses associated with implementing team-based care. TA for this funding opportunity will be provided by Nonprofit Finance Fund (NFF) and John Snow, Inc. (JSI).
Proposed projects must reflect the Foundation’s cornerstones, as our work is grounded in serving Coloradans who have low income and historically have had less power or privilege, putting the creation of health equity at the center of everything we do, and being informed by the community and those we exist to serve.

To be considered for funding, organizations must meet the following criteria:
- Alignment with the Foundation’s cornerstones that advance our efforts to bring health in reach for all Coloradans
- Be a nonprofit clinic or public agency that provides comprehensive primary care services to a significant population of patients who are living on low income and are uninsured and/or underinsured. For-profit clinics will also be eligible to apply for the funding opportunity, provided they meet the criteria and can also demonstrate:
- Urban clinics have an active caseload of at least 40% of Medicaid clients with a low profit margin
- Rural or frontier clinics have at least 30% Medicaid clients
- Provide integrated health care services through teams composed of behavioral, physical and/or oral health care providers*
- Ability to demonstrate strong clinic leadership buy-in and support for planning and implementing team-based care
*Rural clinics interested in applying should contact the Foundation to discuss this criteria.
Preference will be given to organizations that:
Serve patients in rural and/or frontier communities in Colorado. The Foundation follows the Colorado Rural Health Center’s definition of rural and frontier.
- Rural – a non-metropolitan county with no cities over 50,000 residents
- Frontier – a county that has a population density of six or fewer residents per square mile
Examples of eligible organizations:
- Federally qualified health centers
- Rural health clinics
- Community safety net clinics
- School-based health centers
- Community mental health centers with integrated primary care services
If selected, organizations will receive the following:
- Baseline clinic assessment by JSI and NFF focusing on both team-based care implementation and historical financial trends and current financial condition
- Co-development of customized TA work plan to implement or advance team-based care
- Monthly coaching with TA providers, which may be conducted virtually depending on state guidelines related to the coronavirus (COVID-19) pandemic.
- Clinic leadership and staff participation in Regional Collaboratives focused on leaning with peer organizations
Comprehensive primary care: The Foundation has adopted the Institute of Medicine’s definition which is “primary care is the provision of integrated, accessible health care services by clinicians who are accountable for addressing a large majority of personal health care needs, developing a sustained partnership with patients, and practicing in the context of family and community.” Furthermore, integrated refers to physical, behavioral and oral health and the Foundation’s definition also encompasses reproductive health services provided within the integrated setting.
Team-based care: Defined by the National Academy of Medicine as “the provision of health services to individuals, families, and/or their communities by at least two health providers who work collaboratively with patients and their caregivers – to the extent preferred by each patient – to accomplish shared goals within and across settings to achieve, coordinated, high-quality care.” There are five core principles: shared goals, clear roles, mutual trust, effective communication and measurable processes and outcomes.
We often partner with third-party evaluators, contractors and other organizations over the course of our work with applicants and grantees. Your application and its attachments may be shared with these individuals or entities during the review process and grant cycle. All third-party organizations partnering with the Foundation have signed a confidentiality agreement and will not use or share the information for purposes outside of the scope of work specific to the grant application or grant award. Our funding opportunities focus on specific needs aligned with a particular priority within our focus areas. These vary for each funding deadline (Feb. 15, June 15 and Oct. 15). Applicants will be required to address detailed criteria specific to that funding opportunity. Grant funding is highly competitive within the pool of applicants for each individual funding opportunity.
