Project Description
Work with multiple collaborative partners, adapting and enhancing efforts to increase the number of Larimer County residents insured via Medicaid, CHP+, and the Connect for Health Colorado Marketplace. The grant will support the work of Larimer Health Connect Guides' outreach and in-person assistance beyond the second open enrollment period, and anticipates the enrollment of 900 residents in three years.
