Project Description
Support Community Health Centers (CHCs) in further transforming their care models, particularly related to team based care, care coordination, telehealth, integration of behavioral health, and social determinants of health. The APCM Learning Network will build on previous initiatives and interests of The Colorado Health Foundation which have advanced team-based care, access to care, integrated behavioral health, and addressing social determinants of health.
