Work with primary care practices on the priority initiatives of the statewide delivery system and payment reform collective impact initiative, known as Better Care, Better Costs, Better Colorado. The initiatives are transitions of care; reducing ED visits; and facilitating integrated and coordinated primary care. Additionally, contribute to the core building blocks identified as necessary for statewide delivery system and payment reform.
