Improve access to healthcare and health outcomes for vulnerable Colorado youth by increasing (a) universal, comprehensive screening within School-Based Health Centers (SBHCs) and their related community clinics; (b) coordination between SBHCs, community clinics, and schools; and (c) percentages of students served by SBHC and related clinics using data to engage school and student stakeholders to inform health promotion efforts.
