Team-Based Care at New Pediatric Clinic Co-Located at Commerce City Community Campus

Relocate and expand the Kids First Health Care main pediatric clinic to the newly created Commerce City Community Campus (C4), a co-located network of supportive service agencies. The new medical director, new behavioral health practitioner and other staff will work to maximize the clinic's team-based design to deliver integrated care and partnerships with other C4 agencies, and will extend the team-based model to include coordinated client access to a variety of other child and family services.
